The reason is simple: for weight management this is a self-pay prescription, and every clinic sets its own fee. There is no single national price a patient pays. This article sets out our own fees, what is and is not included, and how published figures elsewhere are put together.

People using the medicine for the first time normally start on 2.5 mg. In Japan the dose moves to 5 mg after four weeks, and any increase after that is the doctor’s decision. So the dose is not something you choose from a menu; the doctor decides it after the consultation.

Why the figure differs from clinic to clinic in Japan

  • In Japan, Mounjaro is approved for type 2 diabetes. Use for weight management is off-label, is not covered by public health insurance, and is paid for by the patient.
  • Fees for self-pay care are set by each medical institution. That is why the same dose shows a different figure at different clinics.
  • When you compare, check what the number refers to: one pen or one month (four pens), whether a consultation or first-visit fee is charged separately, and whether tax is included.
  • As a diabetes medicine, Mounjaro does have an official price set by the government for use under public insurance; that is a reference price for the product, not the fee a self-pay patient is charged.

A manufacturer’s list price is not what most patients pay at the counter, and a range advertised by clinics is not a national price. Check the current figure with the pharmacy or clinic you would actually use.

Why the numbers differ: systems, not the medicine

Mounjaro is the same medicine wherever it is dispensed. What a patient pays is shaped by whether it is covered for their diagnosis, how prices are set in that country, the dose, and the exchange rate. None of that is a judgement on any one country’s pricing.

Cost is not the only thing to weigh

Setting our fees out clearly is not the same as advising you to come to Japan for medicine. If you live abroad, there are solid reasons to be treated at home:

  • If a side effect appears, the doctor who can see you promptly is the one where you live.
  • The dose is adjusted slowly over several months, and that needs a doctor who follows you throughout.

Yes. At the clinic we accept cash and major credit cards; for online prescriptions you pay by card through a personal payment link. You are charged in Japanese yen, and the final amount depends on your card issuer’s exchange rate and fees.

Can I buy it tax-free?

No. Under Japan’s tax rules, medicine prescribed by a doctor at a medical institution, including self-pay care, is outside the tax-free and tax-refund scheme.

Can I use Japanese health insurance?

Not for a weight-management prescription. Even if you hold a Japanese health insurance card, this use is self-pay.

Can I buy Mounjaro at a pharmacy or drugstore in Japan without a prescription?

No. Mounjaro is a prescription-only medicine in Japan. It is prescribed by a doctor after a consultation; it is not sold over the counter.

Can I take the three-month plan home?

The three-month plan is 12 pens. Whether you can carry that depends on your destination: several countries allow up to about three months’ supply with a prescription, and some allow less.
